LEMBARAN PENJELASAN KEPADA CALON SUBJEK PENELITIAN
Selamat pagi/ siang Bapak/ Ibu pada hari ini saya dr. Faisal Bustami akan
melakukan penelitian terhadap Bapak/ Ibu dengan penelitian berjudul “HUBUNGAN
MORFOLOGI BLEB TRABEKULEKTOMI DENGAN TEKANAN INTRAOKULAR
PADA PASIEN GLAUKOMA PRIMER”.
Penelitian ini bertujuan untuk mengetahui hubungan morfologi bleb trabekulektomi
dengan tekanan intraocular setelah dilakukan operasi trabekulektomi. Untuk itu kepada
Bapak/ Ibu yang bersedia mengikuti penelitian ini nantinya akan diharuskan untuk mengisi
surat persetujuan bahwa Bapak/ Ibu ikut dalam penelitian, mengikuti pemeriksaan Tekanan
intraokular serta bersedia didokumentasi bola mata selama kontrol pada hari ke-1, ke-7, ke14 dan hari ke-30 setelah dilakukan operasi trabekulektomi.
Bapak/ Ibu yang ikut penelitian berhak untuk menolak dan tidak ikut serta dalam
penelitian ini. Bapak/ Ibu yang bersedia mengikuti penelitian juga berhak apabila sewaktuwaktu Bapak/ Ibu ingin mengundurkan diri dari penelitian ini dan akan dijamin kerahasiaan
pribadi dari Bapak/ Ibu.

LEMBARAN PENJELASAN KEPADA CALON SUBJEK PENELITIAN
Selamat pagi/ siang Bapak/ Ibu pada hari ini saya dr. Faisal Bustami akan
melakukan penelitian terhadap Bapak/ Ibu dengan penelitian berjudul “HUBUNGAN
MORFOLOGI BLEB TRABEKULEKTOMI DENGAN TEKANAN INTRAOKULAR
PADA PASIEN GLAUKOMA PRIMER”.
Penelitian ini bertujuan untuk mengetahui hubungan morfologi bleb trabekulektomi
dengan tekanan intraocular setelah dilakukan operasi trabekulektomi. Untuk itu kepada
Bapak/ Ibu yang bersedia mengikuti penelitian ini nantinya akan diharuskan untuk mengisi
surat persetujuan bahwa Bapak/ Ibu ikut dalam penelitian, mengikuti pemeriksaan Tekanan
intraokular serta bersedia didokumentasi bola mata selama kontrol pada hari ke-1, ke-7, ke14 dan hari ke-30 setelah dilakukan operasi trabekulektomi.
Bapak/ Ibu yang ikut penelitian berhak untuk menolak dan tidak ikut serta dalam
penelitian ini. Bapak/ Ibu yang bersedia mengikuti penelitian juga berhak apabila sewaktuwaktu Bapak/ Ibu ingin mengundurkan diri dari penelitian ini dan akan dijamin kerahasiaan
pribadi dari Bapak/ Ibu.
Segala biaya pemeriksaan ditanggung oleh peneliti dan tidak dibebankan pada
