LEMBAR PENJELASAN KEPADA RESPONDEN
Saya bernama Nur’aini Sitorus adalah mahasiswa S2 Program Studi
Pascasarjana Ilmu Kesehatan Masyarakat Fakultas Kesehatan Masyarakat Universitas
Sumatera Utara. Untuk memenuhi salah satu syarat menyelesaikan S2 Ilmu
Kesehatan Masyarakat yang sedang saya jalani, saya melakukan penelitian dengan
judul “Pengaruh Faktor Karakteristik, Budaya, Dukungan Keluarga, dan Dukungan
Petugas Kesehatan terhadap Pemberian ASI Eksklusif pada Bayi di Wilayah Kerja
Puskesmas Teluk Karang Tebing Tinggi Tahun 2015”.
Tujuan penelitian saya adalah untuk mengetahui bagaimana karakteristik,
budaya, dukungan keluarga, dan dukungan petugas kesehatan. Untuk itu dibutuhkan
kerjasama yang baik antara peneliti dan ibu menyusui di wilayah kerja Puskesmas
Teluk Karang. Identitas ibu menyusui yang diberikan akan dirahasiakan dan hanya
digunakan untuk keperluan penelitian ini.
Apabila ibu menyusui bersedia dan menyetujui untuk menjadi responden
dalam penelitian ini, agar kiranya menandatangani formulir sebagai tanda

Saya yang bernama Nur’aini Sitorus adalah mahasiswa S2 Program Studi
Pascasarjana Ilmu Kesehatan Masyarakat Fakultas Kesehatan Masyarakat Universitas
Sumatera Utara. Saat ini saya sedang melakukan penelitian tentang Pengaruh Faktor
Karakteristik, Budaya, Dukungan Keluarga, dan Dukungan Petugas Kesehatan
terhadap Pemberian ASI Eksklusif pada Bayi di Wilayah Kerja Puskesmas Teluk
Karang Tebing Tinggi Tahun 2015. Penelitian ini merupakan salah satu kegiatan
dalam menyelesaikan tugas akhir di Program S2 FKM USU.
Untuk keperluan tersebut saya mohon kesediaan ibu menyusui untuk menjadi
responden dalam penelitian ini. Selanjutnya saya mohon mengisi kuesioner dengan
memberikan tanda checklist dan menjawab pertanyaan dengan jujur dan apa adanya.
Jika ibu menyusui bersedia, mohon menandatangani lembar persetujuan ini sebagai
bukti kerelaan dari ibu menyusui.
Untuk itu dibutuhkan kerjasama yang baik antara peneliti, ibu menyusui yang
ada di wilayah kerja Puskesmas Teluk Karang. Identitas ibu menyusui yang diberikan

LEMBAR PENJELASAN KEPADA RESPONDEN
Saya bernama Nur’aini Sitorus adalah mahasiswa S2 Program Studi
Pascasarjana Ilmu Kesehatan Masyarakat Fakultas Kesehatan Masyarakat Universitas
Sumatera Utara. Untuk memenuhi salah satu syarat menyelesaikan S2 Ilmu
Kesehatan Masyarakat yang sedang saya jalani, saya melakukan penelitian dengan
judul “Pengaruh Faktor Karakteristik, Budaya, Dukungan Keluarga, dan Dukungan
Petugas Kesehatan terhadap Pemberian ASI Eksklusif pada Bayi di Wilayah Kerja
Puskesmas Teluk Karang Tebing Tinggi Tahun 2015”.
Tujuan penelitian saya adalah untuk mengetahui bagaimana karakteristik,
budaya, dukungan keluarga, dan dukungan petugas kesehatan. Untuk itu dibutuhkan
kerjasama yang baik antara peneliti dan ibu menyusui di wilayah kerja Puskesmas
Teluk Karang. Identitas ibu menyusui yang diberikan akan dirahasiakan dan hanya
digunakan untuk keperluan penelitian ini.
Apabila ibu menyusui bersedia dan menyetujui untuk menjadi responden
dalam penelitian ini, agar kiranya menandatangani formulir sebagai tanda

LEMBAR PERSETUJUAN MENJADI RESPONDEN
Saya yang bernama Nur’aini Sitorus adalah mahasiswa S2 Program Studi
Pascasarjana Ilmu Kesehatan Masyarakat Fakultas Kesehatan Masyarakat Universitas
Sumatera Utara. Saat ini saya sedang melakukan penelitian tentang Pengaruh Faktor
Karakteristik, Budaya, Dukungan Keluarga, dan Dukungan Petugas Kesehatan
terhadap Pemberian ASI Eksklusif pada Bayi di Wilayah Kerja Puskesmas Teluk
Karang Tebing Tinggi Tahun 2015. Penelitian ini merupakan salah satu kegiatan
dalam menyelesaikan tugas akhir di Program S2 FKM USU.
Untuk keperluan tersebut saya mohon kesediaan ibu menyusui untuk menjadi
responden dalam penelitian ini. Selanjutnya saya mohon mengisi kuesioner dengan
memberikan tanda checklist dan menjawab pertanyaan dengan jujur dan apa adanya.
Jika ibu menyusui bersedia, mohon menandatangani lembar persetujuan ini sebagai
bukti kerelaan dari ibu menyusui.
Untuk itu dibutuhkan kerjasama yang baik antara peneliti, ibu menyusui yang
ada di wilayah kerja Puskesmas Teluk Karang. Identitas ibu menyusui yang diberikan
