“Tingkat Pengetahuan Ibu tentang Imunisasi pada Bayi

di Puskesmas Pembantu Naga Timbul Tanjung Morawa Tahun 2012”

  Saya, Liliyani Purnama S, mahasiswa tingkat IV Fakultas Kedokteran

Universitas Sumatera Utara sedang melaksanakan penelitian berjudul “Tingkat

Pengetahuan Ibu tentang Imunisasi pada Bayi di Puskesmas Pembantu Naga

Timbul Tanjung Morawa Tahun 2012”. Tujuan penelitian ini adalah untuk

mengetahui seberapa jauh ibu mengerti tentang pentingnya pemberian imunisasi

pada bayi di Puskesmas Pembantu Naga Timbul Tahun 2012.

  Untuk kepentingan pengumpulan data penelitian ini, saya mohon

kesediaan Ibu untuk menjawab pertanyaan di dalam kuesioner ini dengan sejujur-

jujurnya. Setiap data yang terdapat dalam kuesioner ini bersifat rahasia dan hanya

akan digunakan untuk tujuan penelitian ini saja.

10. Imunisasi adalah untuk menyembuhkan penyakit bukan

mencegah penyakit.

  

11. Imunisasi dapat mencegah anak dan masyarakat dari

penyakit menular.

  

12. Vaksin polio adalah vaksin yang diberikan melalui

suntikan di paha.

  

13. Imunisasi BCG bertujuan untuk mencegah 3 penyakit

sekaligus yaitu difteri, pertusis, dan TBC.
