EMERGENCY INSTALASI GAWAT DARURAT DAN INSTALASI
BEDAH PUSAT RSUP. H ADAM MALIK MEDAN

ABSTRAK
Di Indonesia, biaya pelayanan kesehatan semakin meningkat, dan
merupakan masalah utama pelayanan kesehatan terhadap masyarakat. Penggunaan
sistem Indonesian Case Base Group’s (INA-CBG's) ini dilihat belum efektif, hal
tersebut diperoleh dari hasil penelitian yang menunjukkan untuk kasus – kasus
bedah kecenderungan biaya INA-CBG's jauh lebih rendah dibanding tarif rumah
sakit yang dibayarkan. Tujuan penelitian ini adalah untuk mengevaluasi cost of
illness (COI) pasien BPJS di Kamar Bedah Emergency (KBE) dan Instalasi Bedah
Pusat (IBP) RSUP. H. Adam Malik Medan periode April 2016 – September 2016.
Penelitian ini menggunakan metode deskriptif dengan desain cross sectional dan menggunakan pendekatan retrospektif mengambil data dari Sistem
informasi Rumah sakit (SIRS) dan verifikasi keuangan dari bagian verifikasi
askes, periode April 2016 – September 2016 berdasarkan paket INA-CBG’s.
Penelitian ini berdasarkan perspektif sistem pelayanan kesehatan. Data diambil
dari 78 pasien di KBE dan 100 pasien di IBP berdasarkan kode INA-CBG’s yang
sama. Metode analisis biaya yang digunakan adalah COI digunakan untuk
mengetahui biaya terapi yang tertinggi.
Hasil penelitian menunjukkan persentase biaya langsung medis tertinggi di

KBE adalah penggunaan obat/barang medis yaitu sebesar 92,98 % pada kode
INA-CBG’s W-1-20-II (prosedur pada rahim dan adeneksa sedang). Berdasarkan
kode INA-CBG’s yang sama selisih negatif tertinggi di KBE pada kode INACBG’s M-1-60-II (prosedur sistem muskoletal dan jaringan penghubung lain- lain
sedang) yaitu sebesar Rp. -30.739.627, sedangkan selisih negatif di IBP yaitu Rp.
-51.415.422. Berdasarkan kode INA-CBG’s yang sama terdapat perbedaan biaya
obat/barang medis di KBE dan IBP yaitu di KBE biaya obat/barang medis
tertinggi ditunjukkan pada kode INA-CBG’s M-1-80-I (prosedur anggota tubuh
atas ringan) sebesar Rp. 5.119.518, sedangkan di IBP yaitu sebesar Rp.
20.606.927,5.

ABSTRACT
In Indonesia, the costs of health care keep increasing and remain a major
problem of the community. The use of the Indonesian Case Base Group’s (INACBG's) system has been not yet effective, it was obtained from the results of a
study showing that in cases of surgical with those the cost of INA-CBG's is much
lower than the hospital rates payable. The purpose of this study was to assess cost
of illness (COI) of Universal Health Coverage patients in emergency surgery
room (ESR) and Surgical Center Installation (CSI) H. Adam Malik Medan
Hospital period April 2016 - September 2016.
This study used descriptive method with ross - sectional design and
used retrospective approach to retrieved data from Hospital Information System
(HIS) and financial verification from Askes Verification section, period April
2016 - September 2016 based on INA-CBG's package. Data of patients were
drawn from 78 patients data in the ESR and 100 Patients data in CSI based on the
same INA-CBG's code. The cost analysis method used was COI, used to know the
highest therapy cost. This study is based on the health care provider perspective.
The results showed the percentage highest direct medical costs in ESR
on medicine/medical goods usage of 92.98% in the INA-CBG's coded W-1-20-II

(moderate in the uterus and adeneksa being procedures) Based on the INA-CBG'S
code which samases the highest negative difference in ESR in INA-CBG's coded
M-1-60-II (moderate muskoletal system procedures and other conective tissue)
Rp. 30,739,627, While the negative difference in IBP was Rp. -51,415,422. Based
on the same INA-CBG's code there was a difference in costs medicine/medical
goods in ESR and CSI was in ESR the highest medicine/medical goods costs was
shown in INA-CBG's coded M-1-80-I (mild on limb procedure) Rp. 5,119,518,
while the CSI was Rp. 20,606,927.5.
