Assalamualaikum.,
Bersama ini saya mohon kesediaan Saudara/i untuk mengisi data kuesioner yang
diberikan. Informasi yang Anda berikan merupakan bantuan yang sangat berarti dalam
menyelesaikan penyusunan disertasi saya. Oleh karena itu kepada responden, saya sebagai
penulis mengharapkan:
1. Saudara/i menjawab setiap pertanyaan dengan sejujur-jujurnya, dan perlu diketahui

bahwa jawaban Anda tidak berhubungan dengan benar atau salah

Kuesioner ini dibuat sebagai bahan pengerjaan disertasi untuk Program Doktor dari
mahasiswa S3 Program Studi Ilmu Manajemen. Fokus penelitian adalah untuk mengetahui
prilaku dari konsumen organik. Kuesioner ini digunakan untuk menggambarkan sikap, niat
dan perilaku pembelian produk organik. Oleh karena itu, dimohon dari pihak responden untuk
melakukan pengisian kuesioner secara apa adanya berdasarkan kondisi riil yang terjadi.
Kuesioner ini hanya digunakan untuk kepentingan penelitian dan tidak akan
digunakan untuk kegiatan lain. Kuesioner ini dibuat sebagai bahan pengerjaan disertasi untuk
Program Doktor dari mahasiswa S3 Program Studi Ilmu Manajemen, Sekolah Pascasarjana,
Universitas Sumatera Utara. Fokus penelitian adalah untuk mengetahui prilaku dari konsumen
organik. Kuesioner ini digunakan untuk menggambarkan sikap, niat dan perilaku pembelian
produk organik. Oleh karena itu, dimohon dari pihak responden untuk melakukan pengisian
kuesioner secara apa adanya berdasarkan kondisi riil yang terjadi.
Kuesioner ini hanya digunakan untuk kepentingan penelitian dan tidak akan
digunakan untuk kegiatan lain.
