Lembar Penjelasan Subjek Penelitian
Hubungan Kadar MMP-3 dengan Gangguan Pendengaran pada Pasien AR

Bapak/Ibu/Sdr./i yang sangat saya hormati, nama saya dr. M. Edy
Syahputra Nst., Residen Departemen T.H.T.K.L. FK USU. Saat ini saya
sedang melakukan penelitian untuk tesis magister yang berjudul
“Hubungan Kadar MMP-3 dengan Gangguan Pendengaran pada
Pasien AR”. Untuk melengkapi penelitian ini, saya harus melakukan
wawancara dan pemeriksaan pada Bapak/Ibu/Sdr./i. Sebelumnya, saya
mengucapkan

terima

kasih

yang

sebesar-besarnya

kepada

Bapak/Ibu/Sdr./i atas kesediaannya menjadi responden. Perlu saya
jelaskan bahwa penelitian ini akan digunakan semata-mata untuk
keperluan penyusunan tesis spesialis saya dan tidak untuk keperluan
lainnya.
Setelah penyakit Artritis Reumatoid Bapak/Ibu/Sdr./i diperiksa di SubDepartemen Reumatologi Ilmu Penyakit Dalam, Bapak/Ibu/Sdr./i juga
akan mendapatkan pemeriksaan T.H.T.K.L. di Departemen T.H.T.K.L. FK
USU/RSUP H. Adam Malik Medan. Setelah itu, akan dilakukan
pemeriksaan audiometri nada murni 0,25-8.0 kHz dan timpanometri
dengan probe tone 220 Hz.
Untuk keakuratan data dan informasi yang dikumpulkan maka saya
sangat berharap agar Bapak/Ibu/Sdr./i bersedia memberikan keterangan
yang sejelas-jelasnya sesuai dengan apa yang Bapak/Ibu/Sdr./i ketahui,
alami

dan

rasakan

sehubungan

dengan

judul

penelitian

saya.

Bapak/Ibu/Sdr./i dapat berhenti kapan saja apabila tidak berkenan, namun
