Lembar Penjelasan

Saya, Afdhal Putra, mahasiswa semester VI Fakultas Kedokteran
Universitas Sumatera Utara, saat ini tengah melakukan penelitian yang
berjudul “Faktor Risiko yang Mempengaruhi tejadian Obesitas pada Siswa
SMA Yayasan Pendidikan Harapan 1 Medan”.
Dalam penelitian ini, Saudara/Saudari akan diukur berat badan dan tinggi
badannya. Saudara/Saudari diminta melepaskan alas kaki dan meletakan barang
yang ada di dalam kantong seperti dompet dan handphone. Saudara/Saudari naik
ke atas timbangan, kedua kaki seluruhnya berada di atas timbangn dan pandangan
ke depan. Pemeriksa akan melihat angka yang ditunjuk jarum timbangan.
Kemudian alat pengukur tinggi akan digeser oleh pemeriksa sampai mencapai
puncak untuk menentukan tinggi badan.
Selanjutnya Saudara/Saudari diberikan kuesioner yang terdiri dari 36
pertanyaan. Estimasi waktu dalam mengisi kuesioner adalah 5-10 menit. Dalam
pengisian kuesioner diharapan jawablah dengan sungguh-sungguh dan jujur.

Hasil pemeriksaan dan jawaban tidak akan disalahgunakan untuk
kepentingan lain dan akan tetap dirahasiakan. Apabila selama
