  Saya yang bernama Uli Asri Sihotang adalah mahasiswi Program

Studi Kedokteran Universitas Sumatera Utara. Saat ini saya sedang melakukan

penelitian tentang “Hubungan Faktor Risiko Hipertensi dengan Kejadian

Hipertensi pada Masyarakat Pesisir Laut Kecamatan Belawan”. Penelitian ini

merupakan salah satu kegiatan dalam menyelesaikan tugas akhir di Program Studi Kedokteran.

  Untuk keperluan tersebut saya mohon kesediaan Bapak/ Ibu untuk

menjadi responden dalam penelitian ini. Pada penelitian ini bapak dan ibu akan

diukur tekanan darah, tinggi badan, dan berat badan selanjutnya saya mohon agar

mengisi kuesioner dengan jujur dan apa adanya. Jika bersedia, silahkan

menandatangani lembar persetujuan ini sebagai bukuti kesukarelaan.

  Partisipasi Bapak/ Ibu dalam penelitian ini bersifat sukarela, sehingga bebas mengundurkan diri setiap saat tanpa sanksi apapun. Identitas pribadi ibu/
