PEMERINTAH AMERIKA SERIKAT
UNTUK BERBAGAI PROGRAM KESEHATAN DI INDONESIA
TERTANGGAL :
2012
Amendemen Keempat Perjanjian Hibah untuk Berbagai Program Kesehatan di
Indonesia ("Perjanjian") antara Pemerintah Republik Indonesia dan Pemerintah
Amerika Serikat (secara bersama-sama disebut sebagai "Para
Pihak")
tertanggal 15 September 2009.
BAGIAN A: Dengan ini Amandemen Keempat mengubah Perjanjian sebagai
berikut:
1.
Pasal 3, Bagian 3.1 (a) dari Perjanjian, dengan ini diubah dengan
menghapus "Delapan Puluh Juta Tiga Ratus Tujuh Puluh Satu Ribu Lima Ratus
Sembilan Puluh Satu Dolar Amerika Serikat (A.S.) ($80,371,591)" dan
menggantinya dengan "Seratus Enam Juta Tujuh Ratus Empat Puluh Satu
Ribu Tiga Ratus Tujuh Puluh Dolar Amerika Serikat (A.S.) ($106.741.370)".
2.
Lampiran I (Penjelasan) Perjanjian dihapus seluruhnya dan digantikan
dengan Lampiran 1.
BAGIAN B: Kecuali sebagaimana secara khusus diubah atau dimodifikasi
dalam amandemen ini, Perjanjian akan tetap mempunyai kekuatan hukum
penuh dan berlaku sesuai dengan semua syarat dan ketentuan didalamnya.
BAGIAN C: Amendemen Keempat ini dibuat dan ditandatangani dalam dua
rangkap, yang masing-masing dianggap sebagai lembar asli.
BAGIAN D: Masa mulai berlaku: Amendemen Keempat ini mulai berlaku sejak
ditandatangani oleh Para Pihak.
DI HADAPAN PARA SAKSI , USAID dan Penerima Hibah, yang masing-masing
bertindak melalui wakilnya yang berwenang, telah menghasilkan Amendemen
Keempat yang ditandatangani para waki lnya pada tanggal dan tahun yang
tertulis di atas.

FOURTH AMENDMENT TO ASSISTANCE AGREEMENT
BElWEEN
THE GOVERNMENT OF THE REPUBLIC OF INDONESIA
AND
THE GOVERNMENT OF THE UNITED STATES OF AMERICA
FOR HEALTH PROGRAMS IN INDONESIA
DATED:
2012
Fourth Amendment to the Assistance Agreement for Health Programs in
Indonesia ("the Agreement") between the Government of the Republic of
Indonesia and the Government of the United States of America (collectively
referred to as "the Parties") dated September 15, 2009.
SECTION A:
This Fourth Amendment hereby amends the Agreement as
follows:
1. Article 3, Section 3.1 (a) of the Agreement is hereby amended by deleting
"Eighty Million Three Hundred Seventy One Thousand Five Hundred NinetyOne United States (U.S.) Dollars ($80,371,591)" And substituting in lieu thereof
"One Hundred Six Million Seven Hundred Forty One Thousand Three Hundred
Seventy United States (U.S.) Dollars ($106,741,370)".
2. Annex I (Amplified Description) of the Agreement is deleted in its entirety
and the attached Annex 1 is substituted in lieu thereof.
SECTION B: Except as specifically amended or modified herein the Agreement
shall remain in full force and effect in accordance with all of its terms.

SECTION C:
This Fourth Amendment is prepared and signed in duplicate,
each of which shall be deemed to be a counterpart original.
SECTION D:
Effectiveness:
This Fourth Amendment is effective upon
signature by both Parties.
IN WITNESS WHEREOF, USAID and the Grantee, each acting through its duly
authorized representative, have caused this Fourth Amendment to be signed in
their names and delivered as of the day and year above written.
