  Petunjuk

Kuesioner ini terdiri dari dua bagian, pada tiap bagian berisi beberapa butir

pertanyaan. Untuk menjawab pertanyaan-pertanyaan tersebut, Bapak/Ibu diminta

untuk memberi tanda tickmark (√) pada salah satu kotak yang tersedia.

Sedangkan untuk pertanyaan yang memiliki jawaban isian, Bapak/Ibu diharapkan
