LEMBAR PENJELASAN
Saya Niken Ravita Damanik adalah mahasiswa Fakultas Kedokteran
Universitas Sumatera Utara.Saat ini saya sedang melakukan penelitian dengan
judul “Hubungan antara Merokok dengan Kanker Laring di RSUP Haji
Adam Malik Medan”.Adapun tujuan dari penelitian ini adalah untuk mengetahui
pengaruh rokok terhadap terjadinya kanker laring.
Pada penelitian ini, saya akan mengajukan sejumlah pertanyaan mengenai
pola konsumsi rokok kepada responden. Penelitian ini merupakan penelitian
sosial, bersifat sukarela tanpa paksaan, dan tidak akan dikenakan biaya apapun
kepada responden. Setiap data pada kuesioner ini akan dijaga kerahasiaannya dan
tidak akan disebarluaskan. Data ini hanya akan digunakan untuk kepentingan
penelitian saja.
Jika dalam menjawab pertanyaan pada kuesioner responden mendapat
kesulitan dalam menjawabnya, maka responden dapat menanyakan secara
langsung pada peneliti.
