  Perkenalkan saya Emalia Rosalina, mahasiswi yang sedang menjalani pendidikan dokter gigi di Fakultas Kedokteran Gigi Universitas Sumatera Utara. Bersama ini saya mohon kesediaan Bapak/Ibu untuk mengizinkan anak Bapak/Ibu berpartisipasi sebagai subjek penelitian saya yang berjudul: “Hubungan Karakteristik Saliva pada Anak Usia 37-71 Bulan dengan S-ECC dan non S-ECC di Kecamatan Medan Baru”. Tujuan penelitian ini adalah untuk mengetahui hubungan karakteristik saliva (saliva) pada anak usia 37-71 bulan dengan Severe Early Childhood Caries (S- ECC) dan non S-ECC di Kecamatan Medan Baru.

  Perlu Bapak/Ibu ketahui bahwa Severe Early Childhood Caries (S-ECC) merupakan gambaran kondisi karies atau gigi berlubang pada permukaan halus gigi anak usia kurang dari 71 bulan. Kondisi di atas terkait dengan karakteristik saliva (air liur) yang merupakan salah satu faktor risiko terjadinya S-ECC.

  Dalam penelitian ini terhadap anak Bapak/Ibu akan dilakukan pemeriksaan gigi untuk melihat ada atau tidaknya gigi berlubang, selanjutnya akan dilakukan pengambilan saliva (air liur) sebelum dan sesudah mengunyah permen karet.

  Keuntungan menjadi subjek dalam penelitian ini adalah: • Memperoleh informasi mengenai kondisi rongga mulut anak.

  • Mengetahui risiko Severe Early Childhood Caries (S-ECC) yang disebabkan oleh karakteristik saliva (air liur) sehingga diharapkan orang tua dapat menjaga kebersihan rongga mulut anak dengan baik.

  Kerugian menjadi subjek penelitian ini adalah anak diminta untuk mengunyah permen karet dalam waktu cukup lama (± 5 menit) dan ada kemungkinan permen karet tertelan. Jika permen karet tertelan kami akan memberikan air putih untuk diminum oleh anak Bapak/Ibu.

  Jika Bapak/Ibu bersedia, Lembar Persetujuan Menjadi Subjek Penelitian terlampir harap ditandatangani dan dikembalikan. Perlu diketahui bahwa surat kesediaan tersebut tidak mengikat dan Bapak/Ibu dapat mengundurkan diri dari penelitian ini selama penelitian berlangsung.
