A.1.1

PROJECT SUMMARY

Provide a concise summary of your proposal

Two-thirds (67.4%) of Indonesia males, 4 .5% of females and 36.1% of overall adults
(61.4 million) currently use tobacco in smoked and smokeless forms. More than 97
million Indonesian nonsmokers are regularly exposed to secondhand smoke (SHS).
Smoking kills at least 235,000 people annually. Healthcare costs attributed to tobacco
related illnesses amount to 11 trillion IDR each year (1.2 billion USO). Despites these

facts and figures, Indonesia has not acceded the WHO FCTC yet and there is no
comprehensive tobacco control policy at national level.
Efforts are being made to accede to the WHO FCTC. The President held a special
cabinet meeting in July to discuss on FCTC accession. Follow-up meetings are being
planned. Indonesia has successfully implemented a 40% pictorial health warning since
June 2014. Smokefree policy has been expanded to more than 200 subnational
jurisdictions of which about 50% ensured 100% smokefree in all public places and work
places.
The current project aims to sustain the efforts for acceding to WHO FCTC, to improve a
size of pictorial health warning to at least 75%, to improve legal elements for
comprehensive ban of tobacco advertising, promotion and sponsorship, to increase
tobacco taxes and strengthen tax administration and sustain expansion of smokefree at
subnational level.

A.2.1

PROJECT DESCRIPTION

Purpose of the project and measurable objectives


The proposed project aims to strengthen tobacco control at national and sub-national
level by studying the possibility to acceded to WHO Framework Convention on
Tobacco Control (FCTC) and improving current law and regulation related to tobacco
control.

Objective 1: Prepare the readiness of all stakeholders in Indonesia to
Accede to World Health Organization Framework Convention on
Tobacco Control (WHO FCTC)

Objective 2: Improve awareness and obtain commitments of the law makers on

improving legal provisions to ban tobacco advertising, promotion
and sponsorship
Objective 3: Increase a size of pictorial health warning to at least 75% as per the

MOH tobacco control roadmap
Objective 4: Expand smokefree and ban tobacco advertising and promotion

regulation to at least 30 new subnational jurisdictions in Indonesia

Objective 5: Work with Ministry of Finance to increase tobacco taxes

A.2.2 Context, linkages to work undertaken for current Bl grant, available
resources and partners

Health Law 2009 mandated government to establish smokefree environments, health
warning, labelling and educating the risk of smoking. As per the requirements of the
law, government endorsed an Implementation regulation in December 2012 requiring
enforcement of pictorial health warning in June 2014. Ministry of Health has
successfully managed to enforce a pictorial health warning since 24 June 2014. The
compliance rate was found over 95%.
Smokefree implementation has been gradually expanded in subnational jurisdictions.
Currently more than 200 jurisdictions are being implementing smokefree. Among them ,
more than 80 jurisdictions have a comprehensive ban of tobacco smoking in all indoor
public places and work places. Smokefree at subnational has built a momentum of
tobacco control movement in Indonesia for national tobacco control policy and FCTC
accession. Subnational leaders such as Mayors, Regents and Governors played key
roles to establish smokefree regulation to prevent people from tobacco smoke. A
mayors and regents alliance was established in 2011 with 12 Mayors which now has
been expanded to more than 100 cities and districts in Indonesia. The alliance has

shown as one of the key drivers in tobacco control.

Directorate of Non-communicable Disease Control (NCDC) at the Ministry of Health is
the focal point for tobacco control. It has been playing strategic roles to advance
tobacco control as one of the key intervention to control NCO. NCDC directorate has

been working with the International Union Against Tuberculosis and Lung Disease (The
Union) since 2007 to improve tobacco control in Indonesia. Working together with The
Union, remarkable achievements have been made; i) implementation of 40% pictorial
health warning, ii) establishment of a subnational tobacco control in coordination with
Mayors Alliance, iii) enhancement of partnership between civil society and government,
iv) improving partnership between and among government departments and v)
accelerating collaboration with international partners in tobacco control and NCDs.
Ministry of Health is committed to sustain these achievements and advance tobacco
control policy works.
MOH collaborate with Indonesian Tobacco Control Network, Indonesian civil society
and International Partners such as WHO and The Union. In addition, NCDC continued
to working together with related ministries such as Ministry of Finance, related
organization, NGOs and universities to speed up FCTC accession and other tobacco
control interventions. Indonesian Mayors' and Regents' Alliance will remain as the

strategic partners.

A.2.3 Project strategies and activities
Strategies:
1. Building partnership with Indonesian public figures and enhance their
engagement in tobacco control policy advancement
2. Enhancing engagement with national policy makers and advocates and building
their capacity on tobacco control polices
3. Expanding and consolidating sub-national political will through Mayor's and
Regent's Association to improve tobacco control policy development and
national and sub-national level
4. Mobilizing media engagement and partnership

Coordination and collaboration with inter-sectoral and inter ministerial also one
of the challenges to bring the entire sector on the same perspective to succeed
implementation of tobacco control regulation and FCTC accession process.

Strong tobacco industry interference both at the government and parliament
level.

Pro-tobacco groups' lobbying with national and subnational policy makers
hinder advancing tobacco control policies.

A.2.5 Sustainability

MOH has been working together with Ministry of Finance to advance tobacco control
resources to sustain and development of tobacco control and public health program.
One of the successes is establishments of subnational tobacco tax. As per the national
regulation, subnational governments receive a 10% tobacco tax from the central
government. They are mandated to spend at least 50% to health programs including
tobacco control. This mechanism would contribute to accelerate tobacco control at
subnational level in Indonesia.
At the national level, MOH efforts are being continued to establish a health promotion
fund for tobacco control.
MOH has been strengthening tobacco control and NCO control at national and sub
national level. A partnership with the stakeholders, NGO's, universities, civil society
organization, public health advocate and related ministries for tobacco control also
conducted to enhance commitment to support tobacco control.
To sustain the implementation plan, MOH strengthen commitment and enhance
international and national cooperation in the area of tobacco control. Indonesia will
gradually improve and maintain the collaboration with non health sectors to make
sustainable potential partners and collaboration with related sectors nationally and
internationally through the mobilization of financial resources, manpower and
facilitating the transfer technology. The sustainability of the activities will continued
since every sector will support tobacco control based on their area of expertise. MOH
will continue to increase allocation of budget for tobacco control policy change and
enforcement as well health promotion to build public awareness on danger of tobacco
use.

ORGANIZATIONAL INFORMATION

A.2.6 Organisation history and mission

Directorate of NCDC was appointed by Minister of Health as a focal point for tobacco
control. Established on February 2006, Directorate of NCDC has been an active
participant in various national and regional tobacco control activities. NCDC also being

a coordinator for tobacco control in Indonesia. A national tobacco control roadmap was
developed to carry out the activities as planned way. A national health law was
amended in 2009 with some provisions of as smoke-free and pictorial health warning.
Ministry of Health facilitated to establish a Mayors' alliance in 2011 with 12 mayors
which is now expanded to more than 100 subnational jurisdictions.
As the focal point, NCDC/MOH takes a lead role to coordinate and facilitate ASEAN
tobacco control and NCO.

A.2. 7 Objectives from previous grant
Built high level debate and discussion at the President level on FCTC
accession. A policy paper along with draft President Decree on FCTC was
prepared and it was submitted to the President Office.
A 40% pictorial health warning implemented successfully
Expanded subnational smokefree to more than 200 subnational jurisdictions
however they are various stages on implementation. Among them more than 80
cities and districts are implementing 100% smokefree policy that covers more
than 80% million population in Indonesia.
Built policy awareness at government level and also parliament level to stop
tobacco industry interferences. Ministry of Health drafted a national regulation to
prevent tobacco industry interference. It is on approval process.
