PADA PASIEN GANGGUAN GINJAL KRONIK
DI RSUD Dr. PIRNGADI MEDAN
PERIODE JANUARI – JUNI 2015
ABSTRAK
Hipertensi yang merupakan salah satu faktor pemicu terjadinya penyakit
ginjal akut serta penyakit ginjal kronik (chronic kidney disease, CKD) karena
dapat menyebabkan kerusakan pembuluh darah dalam ginjal sehingga mengurangi
kemampuan ginjal untuk filtrasi darah dengan baik. Salah satu indikator penting
agar tercapainya terapi adalah kesesuaian dosis pada pasien gangguan ginjal
kronik untuk meningkatkan kualitas hidup pasien.
Penelitian ini dilakukan di RSUD Dr. Pirngadi Medan pada bulan
September - Oktober 2015. Jenis penelitian ini adalah metode deskriptif
menggunakan desain pendekatan retrospektif. Tujuan penelitian ini untuk
mengetahui kesesuaian dosis obat antihipertensi pada pasien gangguan ginjal
kronik di RSUD Dr. Pirngadi Medan periode Januari - Juni 2015. Penyesuaian
dosis pada pasien dengan kerusakan ginjal didasarkan pada klirens obat pada
penderita dengan perhitungan Cockcroft-Gault. Sebanyak 40 rekam medik pasien
gangguan ginjal kronik yang memenuhi kriteria inklusi dan dijadikan sebagai
sampel.
Hasil penelitian menunjukkan bahwa berdasarkan usia, kelompok usia

terbanyak berada pada usia 56–65 tahun yaitu 40%. Berdasarkan jenis kelamin
didapatkan pada pasien laki-laki sebanyak 21 orang (52,5%). Berdasarkan stadium
yang diderita paling banyak stadium 5 dengan frekuensi 26 dan persentasi 65%.
Golongan obat antihipertensi yang paling banyak digunakan adalah golongan
kalsium antagonis (amlodipin dan nifedipine) sebesar 33%. Tingkat kesesuaian
dosis penggunaan obat antihipertensi pada pasien gangguan ginjal kronik sebesar
92,31%.
Pada penelitian ini dapat disimpulkan bahwa kesesuaian dosis penggunaan
obat antihipertensi pada pasien gangguan ginjal kronik di RSUD Dr. Pirngadi
Medan periode Januari - Juni 2015 masih perlu peningkatan pelayanan peresepan
obat antihipertensi, karena terdapat dua rekam medik pasien tidak memenuhi
syarat sesuai dengan yang direkomendasikan National Kidney Foundation/
Kidney Dialysis Outcome Quality Initiative (NKF/KDOQI).

ABSTRACT
Hypertension that is one of the factors would trigger of kidney disease
acute and kidney disease chronic (chronic kidney disease, CKD) because can in
damage to blood vessels in the kidneys in order to reduce the ability of the
kidneys to filtration blood well. One of an important indicator to the achievement
of the therapy is conformity doses in patients kidney disorders chronicle to
improve the quality of life patients.
The study is done at RSUD Dr. Pirngadi Medan in September - October
2015. The kind of research this is the method descriptive use design approach
retrospective. The purpose of this research to know conformity a dose of medicine
antihypertensive in patients kidney disorders chronicle at RSUD Dr. Pirngadi
Medan January - June 2015. Adjustment doses in patients with kidney damage
based on klirens medicine in people with by calculation Cockcroft-Gault. As
many as 40 record medical patients kidney disorders chronicle which fulfilled the
criteria inclusion and made as samples.
The research results show that based on age, most age groups is aged 5665 years by 40 percent. Based on sexes was obtained in patients male 21 people
(52.5%). Based on stadium suffered most numerous stadium 5 with the frequency

of 26 and 65% percentage. The antihypertensive drug most commonly used is the
calcium antagonist (amlodipin and nifedipine ) by 33 percent. The level of a dose
of the use of antihypertensive drug in patient kidney disorders chronicle is
92,31%.
In this research we can conclude that conformity a dose of the use of
antihypertensive drug on patient kidney disorders chronic in at RSUD Dr.
Pirngadi Medan January - June 2015 still needs to be an increase service
prescribing antihypertensive drug, because there are two patients medical record
did not fulfil the requirements in accordance with that is recommended National
Kidney Foundation/ Kidney Dialysis Outcome Quality Initiative (NKF/KDOQI).
