PRESCRIPTION MEDICATION ERROR ANALYSIS IN
BREAST CANCER PATIENTS GET THE CHEMOTHERAPY
IN ACTION ADAM MALIK HOSPITAL
ABSTRACT
Medication Error (ME) is an error that occurs in the process of treatment
that could be prevented if the causal factors can be identified early. Medication
errors can occur in the phase of prescribing, dispensing and administration. This
study was conducted to determine the percentage of medication error in
prescribing error category, error dispensing and administration errors.
This research was conducted with descriptive methods, and data collection
was done prospectively to data KOP (Patient Drug Card). Criteria include the
study subjects with a diagnosis of breast cancer patients with/without
comorbidities, who are hospitalized in Adam Malik Hospital during the period
March 2014 - May 2014.
The results showed that of 105 sheets KOP with 228 sitostatica given
amount of drug contained medication errors occur only in the category of
prescribing errors that recipe/Patient Medication Card (KOP) is not complete as
many as 91 cases (86.7%), one of the quantity/number of total 42 cases (40%) and
there are no errors in the category of dispensing errors, administration errors.
Based on data on Patient Medication Card (KOP), it can be concluded that the
apparent phase error occurred medication prescribing in patients with breast

cancer at Adam Malik Hospital.

KANKER PAYUDARA YANG MENDAPATKAN TINDAKAN
KEMOTERAPI DI RSUP H. ADAM MALIK MEDAN

ABSTRAK
Medication Error (ME) merupakan kesalahan yang terjadi pada proses
pengobatan yang sebenarnya dapat dicegah apabila faktor-faktor penyebab dapat
diidentifikasi secara dini. Medication error dapat terjadi pada fase prescribing,
dispensing dan administration. Penelitian ini dilakukan untuk mengetahui
persentase kejadian medication error kategori prescribing error, dispensing error
dan administration error.
Penelitian ini dilakukan dengan metode deskriptif, dan pengambilan data
dilakukan secara prospektif terhadap data KOP (kartu obat pasien). Kriteria

subyek penelitian meliputi pasien dengan diagnosis kanker payudara dengan/tanpa
penyakit penyerta, yang dirawat inap di RSUP H. Adam Malik Medan selama
periode Maret 2014 - Mei 2014.
Hasil penelitian menunjukkan bahwa dari 105 lembar KOP dengan 228
jumlah obat sitostatika yang diberikan terdapat medication error hanya terjadi
pada kategori prescribing error yaitu resep/Kartu Obat Pasien (KOP) tidak
lengkap sebanyak 91 kasus (86,7%), salah kuantitas/jumlah sebanyak 42 kasus
(40%) dan tidak ada kesalahan pada kategori dispensing error, administration
error. Berdasarkan data pada Kartu Obat Pasien (KOP), dapat disimpulkan bahwa
jelas terjadi medication error fase prescribing pada pasien kanker payudara di
RSUP H. Adam Malik Medan.
