Analisis Rujukan Pasien BPJS di Rumah Sakit Umum Daerah (RSUD) Hadrianus Sinaga Kabupaten Samosir Tahun 2017
ABSTRAK
Program Jaminan Kesehatan Nasional merupakan salah satu bentuk
reformasi di bidang kesehatan, yang bertujuan untuk memberi kepastian jaminan
kesehatan yang menyeluruh. Sistem Penyelenggaraan kesehatan dalam Jaminan
Kesehatan Nasional dikenal dengan sistem rujukan berjenjang, yaitu pelimpahan
tanggung jawab yang timbal balik terhadap masalah kesehatan secara horizontal
dan vertikal dengan tujuan memberikan pelayanan kesehatan yang bermutu,
efektif dan efisien. Namun, sistem ini belum diterapkan sesuai dengan peraturan
sehingga meningkatkan beban kerja petugas di fasilitas kesehatan sekunder.
Penumpukan pasien rujukan rawat jalan yang terjadi di Rumah Sakit Hadrianus
Sinaga masih didapat bukan atas indikasi medis pasien.
Jenis penelitian yang digunakan adalah metode campuran. Sampel
berjumlah
84 orang diambil dengan teknik purposive sampling. untuk
menggambarkan karakteristik predisposisi (umur, jenis kelamin, pendidikan,
pekerjaan), dan karakteristik kebutuhan (kebutuhan pasien, dan evaluasi klinis) di
Rumah Sakit Hadrianus Sinaga Tahun 2017. Data dikumpulkan dengan
menggunakan kuesioner dan dilakukan peninjauan dengan metode wawancara
mendalam terhadap 5 informan yang terdiri dari Kepala Puskesmas Tuktuk
Siadong, Dokter Umum, Pegawai Puskesmas Tuktuk Siadong, Perwakilan BPJS
Kesehatan dan pasien rujukan BPJS Kesehatan di Puskesmas Tuktuk Siadong.
Hasil penelitian menunjukkan bahwa pelaksanaan rujukan berjenjang dari
Puskesmas ke Rumah Sakit Hadrianus Sinaga belum sepenuhnya terlaksana
dengan baik dan sesuai dengan prosedur. Karakteristik predisposisi dan kebutuhan
menggambarkan bahwa kasus penyakit yang ditemukan di Rumah Sakit
Hadrianus Sinaga masih tergolong pada penyakit yang harus di tangani di FKTP.
Pengetahuan petugas kesehatan di FKTP sudah cukup baik, sikap dan tindakan
petugas kesehatan yang tergolong kurang baik, dalam prakteknya sering tidak
mengikuti aturan yang ditetapkan. Masyarakat belum mengerti prosedur
pelayanan alur rujukan berjenjang sehingga banyak pasien yang meminta rujukan
atas permintaan sendiri dan disetujui oleh dokter di puskesmas dengan indikasi
medis yang tidak sesuai.
Berdasarkan hasil penelitian, maka diharapkan kepada Dinas Kesehatan
Kabupaten Samosir agar memperbaiki manajemen pengadaan fasilitas kesehatan
serta obat-obatan, kepada Puskesmas Tuktuk Siadong supaya mengupayakan
pelayanan yang sesuai dengan standar dan mekanisme sistem rujukan berjenjang.
Kepada BPJS Kesehatan agar memberikan sosialisasi kepada petugas puskesmas
mengenai standar yang berlaku.

ABSTRACT
The National Health Insurance program is one of the reforms in the health
sector, which aims to provide comprehensive health assurance. Implementation of
the health system in NHI known as tiered referral system, namely devolution of
responsibility reciprocal to health problems horizontally and vertically with the
purpose to provide quality health services, effective and efficient. However, this
system has not been applied in accordance with the regulations that increase the
workload of officers in secondary health facilities. Stacking out-patients referral
that occur in hospitals Hadrianus Sinaga still be obtained not on the patient's
medical indications.
The type of research is survey with mixed methods. The samples numbered
84 people were taken to the purposive sampling technique. to description the
influence of the characteristics of predisposing (age, sex, education,
occupation),and characteristic of needs (the perceived needs, and evaluate
clinical) in Hadrianus Sinaga Hospital in 2017. Data were collected using
questionnaires and crosscheck using indepth interview methodstith 5 informants
consists of the Head Of Puskesmas Tuktuk Siadong, the staff of Puskesmas Tuktuk
Siadong, representative of Health BPJS and patient of BPJS in Puskesmas Tuktuk
Siadong.
The results of this study show that the implementation of hierarchical
referral in Puskesmas to RSUD Hadrianus Sinaga has not been yet running well
and accordane to procedure. Description of the characteristics of predisposition
and characteristics of needs that case of disease in Hadrianus Sinaga Hospital
still belonging to the disease must be handled in FKTP. The knowledge of health
workers in FKTP is already good.Attitudes and action of health workers are still
relatively poorly, in practice it often does not follow the rules set. Society still did
not understand well the procedure of hierarchical referral. Society assesed health
service and complicated referral to get the service.
Based on the results of research it hope that the Head of health Office of
Samosir Regency must improve the management of health facilities supply and the
type of medicine and competency based on applied standart, to the Puskesmas
Tuktuk Siadong to seek services in accordance with standards and mechanisms of
hierarchical referral, to the Health BPJS must pay atention to the socialization to
the staff of Puskesmas about the applied standard.
