Pengaruh Faktor Konsumen dan Provider terhadap Pemanfaatan Ulang Poli Bedah di RSUP Haji Adam Malik Medan

ABSTRAK
Pemanfaatan rumah sakit sebagai penyelenggara pelayanan kesehatan tidak
terlepas dari upaya pemenuhan kebutuhan pasien sebagai konsumen dan rumah sakit
sebagai provider. Jumlah pasien yang tidak memanfaatkan ulang Poli Bedah di
Rumah Sakit Umum Pusat Haji Adam Malik Medan ditemukan mengalami
peningkatan yakni tahun 2011 sebesar 7,9 %, tahun 2012 sebesar 21,0% dan tahun
2013 sebesar 40,5%. Penelitian ini bertujuan menganalisis pengaruh faktor konsumen
dan provider terhadap pemanfaatan ulang di Poli Bedah di RSUP Haji Adam Malik
Medan.
Jenis penelitian ini survei explanatory. Penelitian ini dilaksanakan bulan April
sampai dengan Juli 2014. Populasi adalah seluruh pasien yang direkomendasikan
untuk dibedah sampai dengan bulan Juli tahun 2014 berjumlah 1.184 orang dan
sampel berjumlah 102 orang diambil dengan simple random sampling. Data diperoleh
dengan wawancara menggunakan kuesioner, dianalisis dengan regresi logistik
berganda pada pengujian α=0,05.
Hasil penelitian menunjukkan bahwa secara statistik faktor konsumen
(persepsi tentang penyakit, persepsi tentang pelayanan poli bedah, diagnosa klinis)
dan provider (sikap petugas medis, ketersediaan obat dan peralatan medis, fasilitas)
berpengaruh siknifikan terhadap pemanfaatan ulang Poli Bedah RSUP Haji Adam
Malik Medan. Persepsi tentang penyakit merupakan variabel yang paling
berpengaruh terhadap pemanfaatan ulang. Perilaku dalam pemanfaatan ulang poli
bedah lebih besar dipengaruhi oleh persepsi pasien tentang penyakit. Pasien lebih
banyak memanfaatkan poli bedah ketika penyakit yang diderita sudah mengganggu
aktifitas sehari-hari bahkan sampai pada tahap kronis, sehingga pihak provider
khususnya dokter, perawat dan konselur harus berusaha keras agar perilaku pasien
dalam memanfaatkan ulang poli bedah timbul berdasarkan komitmen yang telah
disepakati sebelumnya oleh dokterb dan pasien.
Disarankan kepada : 1) Manajemen RSUP Haji Adam Malik Medan sebagai
provider untuk : a) mengupayakan pelatihan "service excellent" bagi petugas medis,
b) mengupayakan petugas konseling dan ruangan pelayanan konseling secara khusus
untuk menjamin konfidensialitas dan kenyamanan pasien bedah dalam
berkomunikasi, c) mengupayakan penambahan kamar operasi dan sumber daya
manusia, d) mengupayakan peningkatan kualitas lingkungan pelayanan penataan
fasilitas kamar mandi dan WC untuk pasien, 2) Pasien sebagai konsumen untuk a)
mematuhi jadwal bedah yang telah direkomendasikan oleh dokter dan b)
meningkatkan kesadaran untuk menjaga kondisi kesehatan sebelum tiba jadwal
bedah.

It is found out that the number of patients who did not reutilize the facilities at
H. Adam Malik General Hospital increased from 7,9% in 2011 to 21,0% in 2012 and
40.5% in 2013.This condition will give the image that the hospital as the health care
facility is not optimally used.
The purpose of this explanatory survey study conducted from April to June
2014 was to analyze the influence of the factors of consumer and provider on the
reutilization of Surgery Polyclinic at H. Adam Malik General Hospital. The
population was all of the 1,184 patients who have been recommended to undergo
surgery up to July 2014 and 102 of the patients were selected to be the samples for
this study through simple random sampling technique. The data for this study were
obtained through quetionnaire-based interviews. The data obtained were analyzed
through multiple logistic regression tests at α=0,05.
The result of this study showed that, statistically, the factors of consumer
(perception on the disease, perception on the service provider in surgery polyclinic,
clinical dianose) and provider (attitude of medical/health workers, the availability of
drugs and medical equipment, facilities) had influence on the reutilization of Surgery
Polyclinic. The perception on the disease was the most influenzing variable on the
reutilization.
The factor of consumer which had the most dominant influence on the reutilty
of Surgical Policlinic was the variable of perceptionon illness and the least dominant
factor was the factor of provider. It is recommended that the management of the
hospital a) provide training in communication for health care providers, b) provide
counseling personnel and accomodation for specific cunseling, c) attempt to write the
prescription whih is referred to National and Hospital Formularium, d) improve the
bathroom facility, e) improve the role of PKRS (hospital health promotion), and f)
add Operational Rooms and human resources.
