Western I ndonesia National Roads I mprovement Project 4 - 6 Details of the Contract Package are shown in ANNEX P-12 Executive Summary Report . Package No. 13 : Sp. Rukis Tj. Kemuning The construction activities of this package are currently concentrating only at the segment No.3, Sta 43km 56km 13km because the segment No.1, Sta. O km 25km is almost completed except the minor works and the segment No.2, Sta. 25km 43km 18km is still under process for land acquisition and at the end of May 2017 likely to be settled for around 10km. Therefore, this month progress is only 1.61 against the scheduled 8.95 and the accumulative progress as of this month, 48.87 is much behind, -41.21 comparing with the scheduled, 90.08. The Contractor is in process for the extension of time due to unsettled land acquisition. Details of the work progress are shown in ANNEX P-13 Executive Summary Reports. Package No. 14: Painan - Kambang The actual progress of this month, 9.72 is quite huge against the scheduled, 5.08 This month progress is entirely from the progress of Asphalt paving works, 8.89. On the other hand, the progress of preparatory works for asphalt paving works, like pavements widening and shoulder, granular pavement, drainage are mostly zero or very small. Accordingly, the contractor is advised to increase work forces for such preparatory works mentioned the above to maintain the progress of asphalt paving works continuously in coming months as well. ANNEX P-14 Executive Summary Report shows the complete details of the project. Package No. 15: Sibolga Bts. Tapsel As mentioned in the previous report, this package is also much suffering with unsettled land acquisition because the project site is situated in the areas densely populated and the most of the existing roads are very narrow, nevertheless this month progress, 5.9 is fairly achieved comparing with previous months but, the progress as of this month is only 40.57 against the scheduled, 77.50 having significant slippage rate, -36.93. Consequently, unsettled land acquisition, 16.5km is the most important element to be solved earlier. The PPK is currently trying to settle those areas within July 2017. Detailed and work progresses of the contract package are shown in ANNEX P-15 Executive Summary Report . Package No. 16: Seblat Ipuh This month progress has been leaded by Granular Pavement, Drainage, Earthworks and a head progress is maintained as 1.53, comparing the actual progress as of this month, 8.68 and against the scheduled 7.15. On the other hand, progress of structure works, 0.97 is much lagged behind the scheduled, 1.92. ANNEX P-16 Executive Summary Report shows the complete details and work progress of the project. Western I ndonesia National Roads I mprovement Project 4 - 7 Package No. 17 : Sp. Gunung Kemala Pg. Tampak The progress as of this month achieved of 72.73 against the scheduled 75.33 having a little slippage rate - 2.60. The most of land acquisition have been settled except 30 affected peoples scattered along Sta. 0 3km and Sta. 9.45 9.6km and utility relocation also has been settled except 20 Nos of electrical poles. Early June, Asphalt wearing course will be started. ANNEX P-17 Executive Summary Report shows the complete details and work progress of the project. Package No. 19 : Lais Bintunan The contract package is completed on June 30, 2016 and provisionally handed over to the employer and currently under Defects Notification Period until after two 2 years reckoned from the issuance of the TOC Complete details of the contract package are shown in ANNEX P-19 Executive Summary Report . Package No. 20 : Lubuk Alung - Sicincin The progress as of this month is achieved of 54.23 against the scheduled, 67.61 having slippage rate - 13.38. This month progress is 7.45 against the scheduled, 11.97. In case that the contractor could maintain average progress per month, 7 8 which might not be much overburdened to the contractor because the Major remaining works are asphalt paving works, it is expected that the completion on time as of Dec.2017 could be accomplished. ANNEX P-20 Executive Summary Report shows the Contractor s schedule and work progress. Package No. 21 : Lubuk Alung - Kuraitaji This month progress is achieved of 12.50 which is biggest so far and leaded by drainage works, asphalt paving works. Especially, progress of drainage works is 5.43 only for this month. Is very positive sign that the completion on time, as of Dec. 2017 could be achieved. ANNEX P-21 Executive Summary Report shows the Contractor s schedule and work progress.

4.5 Quality Assurance

The DSC will be responsible for quality assurance of materials and workmanship, certifying whether or not contractors have achieved the required results, as well as documentation and reporting based on the financial management plan. They will prepare a Quality Assurance system covering all subprojects, and continuously monitor the effectiveness of their supervision procedures. Following are current problems which were the same problems during the preceding month and issues relative to the execution of the works and quality assurance: Package No.1 : Krui - Biha a Completed. b Currently under defects notification period for two 2 years until January 17, 2018. c Both TFAC findings follow-up formal report has been approved by World Bank on 14 th September, 2016 Package No.2 : Padang Sawah Sp. Empat a Completed. In accordance with the PMM WINRIP deadline follow up report should be completed and approved less than two months from the time the team TFAC visit and according to records World Bank deadline TFAC follow-up report was approve on 22 March 2017. e Based on the letter Ka.Balai III dated April 17, 2017, to the Satker PJN 1 Prov. West Sumatra that no later than April 25,2017, the follow-up report on the findings of TFAC package 02 third visit after PHO must have been submitted to Balai III Padang. West Sumatra that no later than April 25, the follow-up report on the findings of TFAC package 03 first visit, second visit and third visit after PHO must be submitted to Balai III Padang. Package No.4 : Ipuh - Bantal a Even through the contract time have been extended, contractor still failed to complete and turned over the Project to the Employeer on the revised target date of completion on April 11st , 2016. b Contractor under liquidated damages. c Substantially completed, Taking Over Certificate issued by DSC on May 31 st , 2016. d Currently under defects notification period for two 2 years until May 17 th , 2018.
