7.1.5.1 Smoking Prevalence

Frequencyigures,absoluteandpercentage,aswellasoddsratioswitha95conidence interval [52] alsosmokeORbetween2.1and5.7 [6,72,78–80] .Womenwhosmokearemorelikelyto haveahistoryofdepression [81] .Amongdepressivepatients,however,therearenogender differencesintermsofsmokingprevalence [6] .Among272NewZealandwomensmokers 20yearsofage,63.2hadconcurrentpsychiatricdiseaseandonly22.7werepsychiatri- callynormal.Astudyinvolvingover3,200patientsdemonstratedthatallofthe74whohad experiencedadepressiveepisodeintheirliveshadalsobeensmokers,whereasonly53of thepsychologicallyhealthypatientshadsmokedandthattooonlyonce [6] .7.1.5.2 Peculiarities of the Smoker’s Personality
Themotivesforsmoking [82] ,expressedbybothsexessufferingfromadepressivestate morefrequentlythanbynon-depressives,arethehopedforstimulationcorrelatingpositively withthedegreeofdependence,sedationirrespectiveofthedegreeofdependenceand masteryofnegativeemotionalsituations.Depressivepatientsshowedahigherrateofdepen- denceand,inconnectionwiththis,reducedjoiedevivreanhedonia.Anincreasedfrequency ofdepressiveepisodesinthespringoccursonlyinsmokers,i.e.itisnotseeninnon-smokers [83] .Smokersofbothsexesinapsychiatricclinichadahighersuicideratethanacorrespond- inggroupofnon-smokingclinicpatients.Aprevioussuicideattemptwasfoundtwiceas ofteninthemedicalhistoriesofsmokersthaninnon-smokers,andmoderatetoseveresuicide ixationswereexpressed1.43timesmorefrequentlyamongthesmokersthanamongthenon- smokinggroup [84] .Thereforeaninteractiverelationshipappearstoexistbetweensmoking anddepression:smokingleadstoahigherrateofdepressionanddepressionpredisposes peopletosmoke [85,86] .Theseinteractionsalsoapplytoadolescents [87] . Boththosepredisposedtodepressionanddepressivesmokersdisplayedless-effective “cognitivecopingstrategies”thannon-smokersandnon-depressivesintermsofWaysof RespondingWORtests,leadingtoanattemptatadditional“cognitivebehaviouralther- apy”aspartofwithdrawaltherapyforsmokersathighriskofdepression [88] .Anycon- nectionbetweenadispositiontopsychiatricallyrelevantdisturbancesmood,anxietyor substance-abusedisordersandtherapeuticsuccesswasrejectedonthebasisofcombined withdrawaltherapywithluoxetineplusbehaviouraltherapy,sothataconnectionbetween moodandunsatisfactorytreatmentoutcomewasonlydetectedinsmokerswithoutany previouspsychiatricdisease [89] .7.1.5.3 Reasons for Increased Cigarette Consumption
Inviewofthefactthattobaccosmokecontainsanumberofpsychoactivesubstancesin additiontonicotine,itisconceivablethatthesesubstancesinteractwiththenicotinerecep- tortoincreasethereleaseofneurotransmitters.Thesecretionofmanyneurotransmitters noradrenaline,serotonin,dopamine,acetylcholine,g-amino-butyricacidandglutamateis increasedbythebindingofnicotinetocentralnicotinereceptors.Changesinserotonin 5-HTformationandsecretioncausedbychronicsmokingarealsounderdiscussion [90] .Parts
