7.5.2 Gastrointestinal Ulcers
Pepticulcersshowaparticularassociationwithsmoking,asdemonstratedbyawealthof publicationsintheliteraturefromthe1950stothe1980s.Accordingtoonemeta-analysis, 24ofallpepticulcersareattributabletoNSAIDs,48toHelicobacterpyloriand23 to cigarette smoking [309] . Women who smoke are twice as likely as non-smokers to developpepticulcers,anditisestimatedthatapproximately20ofincidentpepticulcer casesamongUSwomenareattributabletocigarettesmoking [310] .AlargerPolishstudy conductedineightdifferentregionsunderlinestheseindings [311] ,althoughotherauthors considersmokingunlikeHelicobacterpyloriinfectionnottobeanindependentfactor forulcerdevelopment [312] . Onthebasisofendoscopyindings,anassociationhasbeenreportedbetweencigarette smokingandHelicobacterpyloriinfection [313] ,withtheresultthatincreasedsusceptibility toHelicobacterpylorimaybeassumed.GastritiscausedbyHelicobacterpyloriorbysmok- ingisassociatedwithreducedconcentrationsoftheintragastricepidermalgrowthfactor EGF,asubstancefoundinthemucosaandproducedinincreasedquantitieswhenulcers develop;however,thisgrowthfactordidnotplayaroleinthepathogenesisofduodenalulcer [314] .ExperimentalanimalworkindicatesthatsmokingreducesEGF-inducedangiogenesis anddelaysulcerhealing [315] .Inaddition,smokingdepressesgastricmucosalbloodlowas wellastheproductionofNObyinhibitingNOsynthase [316] . OnestudyfromSpaininHelicobacterpylori-positivepatientsdidnotrevealanyasso- ciationbetweenHelicobacterpyloriinfectionandalcoholconsumptionorsmoking [317] . Smokingwasalsonotanadditionalriskfactorfordyspepsia [318] .Wheresmokershad successfullyundergonetherapytoeradicateHelicobacterpylori,cigarettesmokingdidnot increasetherecurrenceofpepticulcers [319] . Duodenalulcersarealsoprovokedbysmoking,andsmokershavebeenreportedto havemorerelapsesandbleedingepisodesthanex-smokersornon-smokers63.3vs.31.2 vs.34.5forrelapses.Ulcerbleedingoccurredinsmokers,butnotinresponsetonicotine intake [320] .Ithasbeenestablishedthatsmokingencouragesthedevelopmentofduode- nal ulcers by inhibiting duodenal mucosal bicarbonate secretion, an important defence mechanismagainstacidandpepticdamage [321] .Moreover,serumpepsinogenIlevels areelevatedinsmokersbecauseofaugmentedpepsinsecretorycapacity [322] . Experimentalanimalworkhasshownthatsimultaneouscigarettesmokingmarkedly impairstheabsorptionofcimetidine,thusattenuatingitseficacy [323] .Thesmokingsta- tusofpatientswasfoundtobeunimportantfortreatmentwithomeprazole [324] .This indinghasbeenconirmedinclinicaltrialsinpatientswithduodenalulcers [325,326] .7.5.3 Cancers of the Gastrointestinal Tract
AccordingtoaPolishstudyin741gastriccancerpatients,smokersofnon-iltercigarettes hadanincreasedriskforgastriccardiacancersOR=3.72;CI:1.35–10.23,compared withcontrols [327] .Otherauthorshavealsoreportedthatsmokersareatincreasedrisk, particularlyfortumoursintheloweroesophagusgastriccardia [308,328–331] ,andthe numberofcigarettessmokeddailyisalsoimportantfortheincreasedlevelofrisk [330] . Comparedwithnon-smokers,currentsmokersof80ormorepack-yearswereatsubstan- tiallyhigherriskforsquamouscellcancerOR=16.9;CI:4.1–69.1 [332] ,andtherisk wasconsistentwithbodymassindexBMI:themorepack-yearsofsmokingandthe highertheBMI,thegreatertherisk.However,smokingisalsoregardedasariskfactorfor adenocarcinomasinthedistalgastricregionandfordiffusegastriccancers [331] .Among patientswithadenocarcinomasandmalignantcarcinoidsofthesmallintestinewhohad alreadyexperiencedulcerdiseasen=36,comparedwith52-healthycontrols,theodds ratioforcigarettesmokingwas4.6CI:1.0–20.7 [333] ,withtheresultthatacausalrela- tionshipmaybeassumed.Inmostofthestudiestheriskwasfurtherincreasedbysimulta- neousalcoholspiritsintake [328] .InonestudyfromRussia,forexample,heavyvodka consumptionhadapotentiatingeffectforthedevelopmentofcancerofthecardiainmen OR=3.4;CI:1.2–10.2andforcancerofsitesotherthanthegastriccardiainwomen OR=1.5;CI:1.0–2.3 [334] .Whilecarcinomasoflowersectionsofthesmallintestine havebeenlinkedwithsmoking,nostudieshavebeenconductedtosupportthis [335] .7.5.4 Pancreatic Cancer
Accordingtoseveralprospectivestudies,smokingdoublestheriskforthedevelopmentof pancreaticcancerinbothmenandwomen [336] .K-rasmutations,associatedwithalcohol consumption,organochlorinesandsmoking,havebeenimplicatedinpancreaticcarcino- genesis [337] . Theriskisincreasedivefoldinpeoplesmoking40cigarettesdaily.Intermsoftumour development,smokersdonotattaintherisklevelofnon-smokersuntil10–15yearsafter smokingcessation [336] .Asalsowithurinarytractcancer,cadmiumhasrecentlybeen suggestedasafactorinvolvedinthedevelopmentofpancreaticcancer:duringsmoking, thisnon-essentialmetalisabsorbedintraceamountsandisknowntoaccumulateparticu- larly in the pancreas [338] . An Italian study has now shown that compared with non- smokers,cigarettesmokersare17.3timesmorelikelytobefoundinthegroupwithchronic pancreatitisand5.3timesmorelikelytobefoundinthosewithpancreaticcancer [339] .Parts
