Dengan hormat, Perkenalkan, saya Ignasius Bhagas Pawitra Adhisakti, mahasiswa Psikologi
Universitas Sanata Dharma, Yogyakarta. Sehubungan dengan penelitian yang sedang saya lakukan dalam rangka penyelesaian tugas akhir saya skripsi, perkenankanlah saya
memohon izin dan kesediaan bapakibu untuk mengisi angket yang saya lampirkan berikut ini.
Angket ini berisi sejumlah pernyataan yang terdiri dari beberapa bagian. bapakibu dimohon untuk mengisi dengan angket ini dengan sejujur-jujurnya, sesuai
dengan keadaan, perasaan ataupun pikiran bapakibu saat ini tanpa harus melihat sisi baikburuknya dan apa yang selayaknya dilakukantidak dilakukan dari pernyataan
tersebut. Bapakibu juga diharapkan dapat mengisi angket ini tanpa dipengaruhi oleh orang lain.
