Empat Including Air Gadang Bridge As of end this month the total overall work progress is 33.73 against scheduled progress of 44.72 incurring a -10.99 slippage. In the previous report of November 2014, the progress showed signs of slight improvement with respect to delayed progress. However, during this period the delayed works abruptly fell behind from - 7.36 to -10.99, mainly due to inadequate manpower, equipment, financial resources, the delayed construction of Air Gadang Bridge which is yet to be started and the very low accomplishment in the asphalt pavement item whichto date only 5 of the 52 of the weighted percentage of the contract had been accomplished. To date, the delayed work is still on a manageable level, but it is already a bit of concern that unless the current problems are addressed immediately, the delayed works will further fall behind in the coming months. ANNEX P-2 Executive Summary Report present a more detailed status of the Contract Package. Package No. 3 : Manggopoh Padang Sawah The current slippage which further slipped down from -17.65 the previous month to -19.63 this month is of much concern due to the very limited remaining contract time left. As of end of this month, only 24.38 of actual progress has been attained in the past twelve 12 months of the contract period. The situation is so alarming with respect to timely completion of the Works considering that almost 50 of the contract time have already elapsed while 75.62 of the works are still to be completed. The main reason for the minimal progress was the delayed works on the asphalt pavement item which accounted for almost 56 of the entire contract as the Asphalt Mixing Plant is frequently out of order. To date only 5.29 of entire work item out of the 55.81 weighted percentage of the contract items is completed. In addition, number of unit of dump trucks is insufficient, low productivity of stone crusher and inadequate manpower for drainage work also contributed to the slow progress. Due to urgency to maximize the output of paving works to attain a modest progress, the Contractor shall maintain the AMP regularly and address and resolve all issues that are presently hampering the progress of the works. A more detailed status of the project progress are shown in ANNEX P-3 Executive Summary Report . Package No. 4 : Ipuh Bantal The current status of this contract package still remain in critical situation as the negative slippage of -24.66 incurred last month further slipped down to -31.53 as of end of this reporting month. The actual progress of only 9.35 since the project started a year ago is of much concern than the slippage. Simply looking at the percentage of work accomplished, it seems that the project is only Western I ndonesia National Roads I mprovement Project 26 on the initial stage of construction, whereas by this end of the 12 th month period, at least 41 of the works should have been completed. The major work item which should have been given more concentration in order to attain a modest progress during the period would have been the drainage works 12.26 granular pavement 18.07 and asphalt pavement 49.27 which have a combine total work value of 79.40 . However, the Contractor was able to accomplished only 1.14 , progress for the three 3 major works item out of the overall month s progress of 1.25 . As agreed during the progress meeting held in Jakarta Bina Marga Office during the middle of the month, the Contractors committed to conduct a test Case for a period of two 2 weeks from December 17-31, 2014 with targeted accomplishment of 4. The Test Case was unsuccessful as only 1.25 progress was attained not in two 2 weeks time, but for the whole month. A high level management meeting between DGH and Contractors was held in Padang during the middle of the month wherein the main agenda were to resolved all issues and mayor problems that hampers the progress of the works. It is expected that in the coming months, the work progress will ultimately offset the previous slippage and be on time at the target date of completion. Shown in ANNEX P-4 Executive Summary Report are the current problem and actions to be taken to attain substantial progress in order to catch-up with the schedule. For contract packages under AWP-2 and AWP-3 which have a combined total of 17 Packages, a brief summary of the actual activities and status as of this reporting period are presented in Table 1.1, page 7 of this Report. 4.5 Quality Assurance

The DSC will be responsible for quality assurance of materials and workmanship, certifying whether or not contractors have achieved the required results, as well as documentation and reporting based on the financial management plan. They will prepare a Quality Assurance system covering all subprojects, and continuously monitor the effectiveness of their supervision procedures. During the month of December 2014, the CTC s Quality Assurance Engineer attended the general coordination meeting on December 22 and 23, 2014 in Padang

4.6 Environmental and Social Considerations

4.6.1 Current Overall Situation A project category is assigned to each package as either Category A, B or C depending on the likely degree of influence on environmental issues with a brief explanation as follows: Category A project is one which will have a significant negative impact on the environment and may have an influence on areas outside the actual project area. A project classified as such requires an AMDAL ANDAL, RKLRPL study. Category B project is one which affects the people or the environment to some extent but with a smaller impact than a Category A. The required environmental analysis is a UKLUPL study only. Category C project has minimal effect on the people or environment and an environmental analysis is not required. SPPL are used to handle any minor problem which may arise. All the twenty one 21 packages require environmental impact investigations. There are a total of fourty one 41 studies required, twenty 20 for environmental impact investigations and twenty one 21 for social studies. The twenty 20 environmental studies need the approval of the Environmental Agency BLH with five 5 of the studies AMDALEIA to be approved by World Bank WB. While the twenty one 21 social studies SLARAP and LARAP require the approval of World Bank prior to implementation. As of end of this reporting period December 2014, twenty 20 environmental impact studies has been approved by BLH, while all of the five 5 studies that requires World Bank approval, Bank already issued No Objection Letter NOL. For the social studies , twelve 12 of the twenty one 21 studies were already given No Objection Letter from the World Bank WB. Details of sub-projects that require environmental and social safeguards and progress are shown in Appendix E. Appendix E shows the Progress of Environmental Safeguard Studies. 4.6.2 Environmental and Social Issues This section outlines the activities of the Social Impact Specialist and the Environmental Specialist during December 2014.

4.6.2.1 Environmental Issues

During the month of December 2014, the RKPPL prepared by Contractors of Packages 2 and 3 were submitted to World Bank, while RKPPL for Packages 1 and 4 are under signing procees by the Contractors and PPK. While awaiting the signing of the documents, CTC informally submitted to World Bank the advance copies of the documents.

1. Monitoring of Environmental Aspects During Construction

CTC provided monitoring forms to DSC for use in monitoring of environmental and social impacts. These forms are to be used every month by the DSC Environmental Specialist. Western I ndonesia National Roads I mprovement Project 29 CTC s environmental staffs actively advises DSC on environmental monitoring, but DSC have not directly carried out monitoring or to implement the Environmental and Social Analysis Management Plan for WINRIP. To date, only the DSC s Site Engineer of Package 1 have regularly submitting the monthly environmental mitigation reports since January to September 2014 although the Reports were not in standard format that was provided by CTC. Reports covering the last three 3 of 2014 are still to be submitted. For the three 3 other packages, the Site Supervision Engineers of Contract Packages No.2, 3 and 4 have never submitted not even a single report since commencement of projects in the early days of January 2014. CTC issued letter to DSC on July 23, 2014 requiring DSC to submit the Reports at each end of the month. Environmental Monitoring Report for the month of December 2014 have not yet been submitted by the Site Supervision Engineers of DSC to CTC.

2. Design Integration, Road Safety Aspects and Monitoring of Environmental Studies

Task 5.8.0 of CTC-2 TOR Implement the Environmental and Social Analysis Management PlanESAMP and paragraf 10.9 of the PMM requires ensuring that environmental management aspects of contracts have been implemented in accordance with AMDAL, UKLUPL, SPPL studies and environmental issues will be integrated in the design and to prepare environmental mitigation on construction stage for contractor Contractor Program for Executing Environmental Management Plan CPEMPRKPPL. Therefore CTC will proposed that staff visit the site of projects subject to studies or issues, and ensure that the plans are being implemented. A schedule for these visits are scheduled as the need arises. During the period December 2014, the CTC s Environmental Specialist conducted verification survey on Package No.8 Bts. Pariaman - Manggopoh, Package No.20 Lubuk Alung Sicincin and Package No.21 Lubuk Alung - Kuraitaji. The recommendations of AMDAL and UKLUPL are not yet incorporated in Detailed Engineering Design DED. CTC will coordinate with DSC and BALAI II regarding this matter.
